Your Admission to ELPIS

Any individual interested in rehabilitation services and meets our admission criteria is welcomed at ELPIS.

ELPIS’s Assessment Criteria has been developed to better understand a client’s drug, alcohol, health and welfare needs and determine whether the services provided meet their specific requirements.

Assessment Criteria

Potential clients wishing to be admitted, please call (02) 9816 1859 or email: info@elpisfoundation.org.au with your interest.

One of our team members will talk through the information that we require to determine if ELPIS is the best option for you.

Clients may be required to provide documentation regarding medications, mental health, Centrelink status and criminal history prior to assessment.

Clients will need to organise their Centrelink benefits prior to admission or be able to meet the required weekly payments.

Applicants who provide information which may later be found to be false or misleading, may be subject to discharge at any time after admission.

General Practitioners, Clinicians & Service Providers

General practitioners, clinicians and service providers wishing to refer clients to ELPIS, need to complete and submit the online Clinician Referral Form*.

* Clients must be aware of the referral and give consent by signing the referral form prior to it being emailed to ELPIS.

Residential Rehabilitation Costs

The ELPIS residential rehabilitation program is affordable if you are on a Centrelink benefit and have a current Medicare card. If this is not the case, please call us to discuss costings on (02) 9816 1859.
